A screening, not a diagnosis

One Beat Health™ is a community heart-health check-in offered through your faith community. It gives you a personalized summary and next steps based on your answers — it is not a medical diagnosis and it is not a substitute for care from a licensed clinician.

If something feels wrong right now

If you are having chest pain or pressure, sudden trouble breathing, weakness on one side of your body, sudden trouble speaking, or you feel something is very wrong: call 911 or go to the nearest hospital.

Talk to a clinician

Bring your results to your primary care doctor, community health clinic, or another trusted clinician. They know your history and can help you decide what to do next. Your results include talking points you can share with them.

Please answer honestly

Your results only reflect what you tell us. Please answer as honestly as you can — it's just for you.

How you may use this platform

  • Take the check-in for yourself.
  • Share your own results with your own clinician or loved ones.
  • Do not attempt to access anyone else's session, results, or event data.
  • Do not attempt to disrupt, scrape, or bypass the security of this platform.

No warranties

We work hard to give useful, warm, accurate information — but this platform is provided "as is." We can't guarantee outcomes, and no content on this site creates a doctor-patient relationship.

Changes

We may update these terms as the program grows. The version in effect at the time you participate is the one that applies to your session.
